Medical Services

Professional nursing services are provided in accordance with the plan of care and are case managed by licensed Registered Nurses who may also be the visiting nurse assigned. Additionally, case managers may assign LPN’s who service cases under the supervision of the RN and assist in the implementation of the plan of care.

Physical therapy services are provided in accordance with the plan of care and are case managed by qualified, licensed physical therapists who may act as the primary case manager in the absence of the nursing discipline. Additionally, PT case managers may assign PTAs (Physical Therapy Assistants) who service cases under the supervision of the PT and assist in the implementation of the plan of care. PT may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.

Occupational therapy services are provided in accordance with the plan of care and are case managed by qualified licensed Occupational Therapists (OT). Additionally, OT may assign qualified OTAs (Occupational Therapy Assistants) who service cases under the supervision of the OT and assist the OT in the implementation of the plan of care. OT may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.

Speech Therapy services are provided in accordance with the Plan of care and are provided by a qualified licensed speech therapist. ST may develop rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.

A certified nursing assistant (CNA) provides varying levels of care and support to physically and/or mentally disabled individuals who are unable to care for themselves. CNAs care for patients of all ages in hospitals, inpatient mental health facilities, and clinics.

HHA services are ordered by the primary case manager (PCM) as appropriate to each case. The referral for HHA service is written and forwarded to the Scheduling department for assignment. HHAs assist with personal care and hygiene, assist with ambulation, transfers and exercise programs, provide basic nutritional support, provide light housekeeping tasks related to the patient.

An LNA provides basic nursing care which includes assistance with bathing, dressing, eating, mobility and toileting. They also take vital signs, obtain specimens, observe and report information, as well as many other tasks.

A RN is a clinician with a nursing degree who has passed a licensing exam.

Our fully-screened and certified personal care attendants are available to assist you with everyday tasks, such as eating, bathing, or light housework. The purpose of companionship care is to offer company and friendship. Companions may help with certain household chores, such as laying out towels before a shower, but their primary purpose is to spend time with the patient. Grown children may want to hire companions to keep an eye on their aging parents, for example.

Companionship care services include:

Everyone enjoys a clean home, but for the elderly or disabled, even simple housekeeping tasks can become unmanageable. If a client’s primary caregiver needs to take a temporary break, our respite care program provides a relief and assistance to client according to the needs of the client or the plan of care established by the client’s physician.

The management of patient’s medications prescribed by the physician and when taken correctly as prescribed, supports the healing of the patient. Knowledge of the patient’s medication and medication regimen at home provides a key element to the physician’s care of the patient.

Alzheimer’s and dementia patients experience memory loss, confusion, difficulty moving and changes in behavior or personality. Our caregivers ensure our clients receive the assistance he or she needs to live a better quality of life.

Alzheimer’s and dementia care includes:

  • Support existing cognitive ability with mental stimulation and social interaction
  • Promote optimal health with meal preparation
  • Assistance with hygiene such as bathing, dressing and grooming
  • Transportation for errands, shopping and doctor’s appointments

We provide non-emergency transportation for our clients to and from medical or other appointments, as well as light errands or visits to family and friends.

Non-Medical Services

Being away from home and family is particularly stressful for patients, but it can also be difficult for their family members who are not beside their bed. Our caregivers can plan and arrange a hospital visit, help you with packing, with arranging transportation and make sure your house is safe during your absence. Our home health caregivers can be perfect companions while your loved ones are staying in the hospital. Once you are settled in, your personal caregiver can stay by your side as much as you need – even 24 hours a day.

As an experienced provider of personal care programs, we provide support to individuals in the comfort of their homes. Some of the services included in personal assistance are:
